Medical Information

Last Updated: Jul. 8, 2012

One of the most important items we need from you is your child’s health record and medical consent form. Because some campers may arrive at camp with a friend or on the bus, it is extremely important to us that these forms are taken care of as soon as possible. You can fill them out online under the link on the left bar called “Document Center.” Don’t hesitate to call us if you have trouble finding the link.

Many campers also require medications during the week. To ensure that your camper receives and takes home the correct medications, please put any medications in a plastic ziplock bag, and label the original packaging with full name, dosage, and directions. Medications will only be accepted if they are in original packaging. You can also enter these instructions online on your child’s health forms in advance and be one step ahead at registration when you talk with the camp Medical Staff Access your online account to complete the medical release forms before arrival. Please go to your account online and click on your camper’s name on the left. At the bottom of the screen under forms, if it is blank or says “incomplete” please view and complete the forms. You can now take advantage of e-signature on this form so you do not need to fax or mail the form to us.
